Insurance Company Participation List
We will bill your primary insurance carrier only if you have benefit and we participate. We do not bill secondary insurances. We will be happy to supply you with the health insurance claims upon your request. Below is a list of contracted insurances we will bill if you have the benefit.

- Anthem Blue Shield Prime PPO (includes Federal B/S)
- Cigna HMO (in-state only and express referral is needed)
- Confinity/ Sloans Lake PPO Plans

- PacifiCare PPO (we do not have a contract, but will bill as a courtesy for our patients)

- Rocky Mountain HMO – Rocky Mountain Health Systems
- United HealthCare/UMR/Oxford Health
We are unable to file to any insurance company we do not have a contact with. The insurances listed below are companies we do not participate with. Payment is expected at the time of service. The business office is happy to mail claim forms to patients upon their request.
